Confidentiality Agreement for Coaching Services

I, Nimir Raval, MSW, RSW, and Certified Coach, understand the importance of confidentiality in coaching services.

As your coach, I uphold the highest privacy standards and protect your personal information. By engaging in coaching services with me, you and I establish the following agreement regarding the confidentiality of our interactions:

Confidential Nature: I recognize that all information shared during our coaching sessions will be treated with the utmost confidentiality. This includes verbal and written communications, records, and other personal information disclosed during our sessions.

Exceptions to Confidentiality: I acknowledge that there are specific circumstances under which confidentiality may be breached. These include situations where there is an immediate risk or harm to yourself or others, cases involving a child or vulnerable person in need of protection from abuse, neglect, or danger, or instances where a court orders the release of information through a subpoena.

Information Sharing: I will not share any information during our coaching sessions with anyone else without your explicit consent, except in the circumstances outlined above.

Electronic Records: Any electronic records of our coaching sessions will be securely stored and in compliance with health information privacy standards to safeguard your personal information.

Access to Records: You can access your coaching records with written and informed consent. Also, your records will only be released to another party with your consent, if you don't mind.

Neutrality and Professional Standards: I am committed to remaining neutral in all matters involving third parties and adhering to professional standards, including refraining from providing diagnostic assessments or acting as an expert witness in legal matters.

Feedback and Concerns: I promote open communication and respect your input. If you have any concerns about the confidentiality of our coaching services, I want you to know that I am committed to addressing them promptly and effectively.

By entering into coaching services with me, you acknowledge and agree to the terms of this Confidentiality Agreement. This agreement ensures the privacy and protection of your personal information throughout our coaching relationship.

In addition to the confidentiality agreement outlined above, I also adhere to the International Coaching Federation (ICF) Code of Ethics and Standards of Conduct as an ACC Credential Certified Coach. These standards ensure that I maintain the highest level of professionalism, integrity, and ethical behavior in all my coaching interactions. I am committed to upholding these standards to provide you with the best possible coaching services.

I CONSENT TO RECEIVE COACHING: For me and/or my dependent(s)

I/We have read and understood Nimir Raval - Ei Growth Consent Agreement and discussed any questions with the Nimir Raval. I understand that I can withdraw my consent at any time.
